Primary Care

The Medical Home Model

Grant Alignment: HRSA Health Center Program

Our Primary Care pillar is built on the Medical Home model ,delivering continuous, coordinated, patient-centered care that prioritizes chronic disease management. Patients are assigned a primary care provider and care team who know their history, track their conditions, and proactively manage their health over time.

Pediatric Behavioral Health

Family Stabilization & Early Intervention

Grant Alignment: SAMHSA Priority Program
 

Our Pediatric Behavioral Health pillar addresses the urgent mental health needs of children and families  before they reach a crisis point. Through early intervention, parent education, and evidence-based therapy, we stabilize families and set children on trajectories of lifelong wellness.

Collaborative Mental Health

Co-Location of Care

Grant Alignment: Integrated Care Model

Therapists and psychiatric providers are physically embedded within our primary care clinics  removing the stigma of seeking mental health services and eliminating the logistical barriers of a separate appointment. This co-location model produces significantly better engagement and outcomes than traditional referral systems.

Population Health Management

Social Determinants of Health (SDOH)

Grant Alignment: Equity Initiative

Healthcare doesn’t happen in a vacuum. Our Population Health pillar addresses the Social Determinants of Health  the non-clinical factors that drive up to 80% of health outcomes. From free patient transportation to food access navigation, we ensure that no social barrier stands between a patient and their care.
