PROGRAMS & IMPACT
The Four Pillars of Care
Each program module addresses documented community health disparities and aligns with 2026 federal and state grant priorities.
01
Primary Care
Chronic disease management and longitudinal care through the Medical Home model — delivering consistent, relationship-based primary care.
02
Pediatric Behavioral Health
Early intervention and family stabilization services for children and adolescents, addressing behavioral health needs before they escalate.
03
Collaborative Mental Health
Therapists embedded within the medical clinic, reducing stigma and wait times while delivering fully integrated whole-person care.
04
Population Health Management
Addressing Social Determinants of Health — including free patient transportation, food access, and housing navigation — so no barrier is left unaddressed.
Board Governance
Independent board of directors with professional credentials across medicine, finance, and law — available for grantor review.
Policy Transparency
EEOC Policy, Conflict of Interest Policy, and Operating Reserve Policy — all available for download on our Governance page.
Fiscal Accountability
990s on file, audited financials, and a documented operating reserve demonstrating organizational sustainability
